Summer Bridge Registration

Registration Forms and Checklist


Deadline: June 1
Please complete all forms and submit as soon as possible and no later than June 1. You will receive all of these forms in a packet in the mail. You may also download the forms here. Please mail all forms to the address listed at the bottom of this page.

Mandatory forms for all attendees:

  1. Enrollment Agreement for Summer Bridge (download)
  2. One-time Tuition Deposit (make payment online)
  3. One-time Health Clearance form (download)

Housing forms

These forms must be filled out if you plan to stay in on-campus housing. (note: on-campus housing is not mandatory for summer bridge.)

  1. Housing Contract (download)
  2. $300 one-time housing deposit for year 2010-2011

Recommended forms:

It is recommended that you fill these out and submit:

  1. FAFSA- Please complete the FAFSA as soon as is feasible if you have not already done so. This will allow us to assist you with financial aid.
  2. AAP Intake Form - Please fax your completed AAP Intake Forms to: (808) 739-4671 (download)
  3. Shuttle request form, if you need transportation from the airport.
