O&P Instructions

Being Placed by the Field Services Office

Please complete the following items upon registering for the OP course. All requirements must be in Field Services prior to the start date of your OP course.

  • OP Request Form - fill this out and send it to edu-fieldservices@chaminade.edu or fax to 808/739-4607 immediately upon registration for your OP course.
  • Insurance Form/HSTA Membership - go to www.nea.org and sign up to be a student member
  • Background Check/Fingerprinting - the Field Services Director will submit your name to the DOE for background/fingerprinting clearance and will contact you if you will need to update your background/fingerprinting clearance.
  • Complete the FAQs by reading the Field Experience Manual.
  • TB Clearance - must be cleared through the end of the OP course. Please submit a copy to Field Services upon registering for the OP course. Update as necessary.

After receiving confirmation of your placement by the Field Services Director, you are to:

  • Call or email the OPT as soon as possible to "connect" and set a time and date for your first visit.
  • Always communicate any changes regarding time, plans, etc, to your OPT as soon as possible. When in doubt, communicate! Check your student email at least 3 times per week. It is preferred to set a schedule and stick to it.
  • Dress professionally! Be sure to follow the school's dress code.
  • Remember you are an emissary of Chaminde's Education program.
  • Contact the Principal on the first visit to introduce yourself and confirm your assignment.
  • Keep the Attendance Form up to date, for keeping record of your hours. It must be completed, signed and returned to your course instructor at the end of your practicum. Be sure to bring all forms for the host teacher on the first visit.
  • Act like a professional! This may be where you end up teaching, or get hired for a job. Make a good impression.

Evaluation Forms

  • Evaluation forms will be sent to you and your host teacher via email.

Last Notes

  • It is a requirement for you, as a visitor, to check in with the school's office staff. You must wear your CUH ID in a clear sleeve, attached to your shirt or on a lanyard around your neck at all times during your OP visit.
  • Each time you arrive and leave the school campus, be courteous. Your congeniality with school staff and faculty is always noted!
  • The preferred arrangement for O & P attendance is a schedule of two consecutive hours a week over a ten-week period (for courses requiring 20 hours of O & P). If circumstances make this particular scheduling impossible, be sure to confer with your OPT and course instructor regarding another acceptable arrangement.
  • Please be in communication with the Field Services Director if there is a problem: edu-fieldservices@chaminade.edu.
  • You will be notified via CUH student email of your placement after Field Services has received all parts of your O & P requirements. You do not need to follow up.

Full-Time Teachers Placed "In-House"

Please complete the following items upon registering for the OP course. All requirements must be in Field Services prior to the start date of your OP course.

  • Notify Field Services via email at edu-fieldservices@chaminade.edu if you are already a full-time employee in a school. This lets Field Services know you don't need to be placed.
  • Complete the FAQs by reading the Field Experience Manual.
  • TB Clearance - must be cleared through the end of the OP course. Please submit a copy to Field Services upon registering for the OP course. Update as necessary.

Forms you will need:

  • Acceptance Agreement - if your are observing other teachers within your own school, you will only need to complete the "In House" Agreement form immediately (to be signed by the Principal/Director). This is due to Field Services prior to the start of the OP course.
  • OP Timesheet - You will need this to turn in to your CUH instructor and Field Services Director. This is done prior to the end of the term.

Please note: there will be no compensation given for "in-house" OP.
