HONOLULU – November 6, 2009–A report, containing confidential
information including social security numbers of Chaminade University
students, was found November 4, on the university’s website. Upon
discovery, all files in question were immediately taken down, and all links
disabled.
The report was inadvertently placed on obscure, though publically accessible
web pages. The information was accessible for approximately eight months,
although there is no evidence of its use. The university estimates that
personally identifiable data for 4,500 students were in the report. Those
affected are among persons who attended the university as undergraduate
students from 1997 to 2006.
The university has conducted a thorough investigation into the cause, which
has been determined to be human error rather than any malicious intent.
The university is putting into place procedures designed to prevent a
recurrence of this type and will be providing additional training to its staff
regarding the protection of personal information.
Efforts to contact affected parties are underway. Parties affected will be
asked to monitor and review their credit report, which is available for free at
Annualcreditreport.com or by calling (877)322-8228.
For more information, a Chaminade email address has been set up at
infosecure@chaminade.edu, as well as a webpage at
