Test Administration Policy
The AAP/SSS offers Test Administration for students that: 1) have documented accommodations on file with the Counseling Center, 2) need to make-up an exam 3) take an exam on a different date than the rest of the class. All requests for Test Administration through the AAP/SSS must be submitted by an instructor.
- To arrange for an exam to be administered, Instructors are required to submit paper copies of exam with a Test Administration Form (TAF), these forms can be obtained from our office(s) in the Student Support Services building, or printed from our website at, http://www.chaminade.edu/student_life/sss/aap.php Please submit exam and required documentation to a staff member at least two hours prior to a testing session. We reserve the right to deny services and/or reschedule sessions.
- At the top of all exams, please include: name of student, course number & title, and name of instructor.
- Staff will be present in the AAP/SSS testing area. Please note that our area may be crowded and/or loud, due to tutoring and other services. Disposable ear plugs will be distributed upon request.
- We are unable to provide a private testing space for students unless there is a required accommodation approved through the Counseling Center (please request reservation 24 hours in advance).
- Test Administration is available Monday through Friday during regular business hours (M-F, 8:30 a.m. - 4:30 p.m.). Faculty and students must plan ahead to allow enough time to start and complete an exam, office hours will not be extended!
- All students utilizing this service, in the AAP/SSS, must read and sign the "Test Administration Procedures & Guidelines".
- Students are expected to remain in the testing area until the exam is complete. If a student leaves the center in the middle of session: the exam will be collected by a staff member, the instructor will be notified, and the exam will be held in the office until it is picked up by the instructor.
- Instructors must pick up all exams. If an instructor is unable to do so, s/he must notify an AAP/SSS staff via telephone or e-mail (see TAF for contact information) to authorize a proxy to sign for exam. If exams are not picked up by the last day of finals, the staff will return materials via interoffice mail to the division office.
- If a student is caught cheating during an exam, the exam will be collected by a staff member, and the instructor will be notified immediately.
- The AAP/SSS is unable to accommodate Test Administration for an entire class. Instructors should make arrangements with her/his individual department staff if s/he is not able to personally administer an exam.

Test Administration Procedures & Guidelines
Before we can administer your exam, carefully read through the following guidelines below and make sure you thoroughly understand them. If you need assistance in understanding our procedures & guidelines, please feel free to ask at this time. Once the exam is administered you are bound to this agreement.
- You will be required to show a Chaminade University of Honolulu Student ID Card.
- Personal belongings (i.e., all bags, cellular telephones, books, notes, and calculator) and other items not required for the exam will be placed under the desk.
- Due to limited space, we do not have a private room in which you can take your exam.
- There will be no breaks once your exam has been administered. No exceptions!
- During the exam, please do not ask others for help. If you have a question, please bring it to the attention of a staff member.
- At no time are you allowed to use a computer, dictionary or other resources to assist you in your exams, unless specified by your instructor.
- If you are caught violating any of the testing guidelines, the AAP/SSS staff has the right to confiscate your exam. No warnings will be given. Violations may include, but are not limited to: Materials not authorized for the exam; Taking unauthorized breaks; Talking; CHeating/Academic Dishonesty; Exceeding time limit.
- In the event your exam is confiscated, a staff member will document the incident and notify your instructor. In addition, you will no longer be able to take exams in the AAP/SSS center.
