Summer Bridge Registration

Registration Forms and Checklist

Deadline: May 30th

Please complete all forms and submit no later than May 30th. You will receive a packet of these forms
in the mail. You may also download the forms here. Please mail all forms to the address listed at the
bottom of this page.

Mandatory forms for all attendees:

  1. Enrollment Agreement for Summer Bridge (download)
  2. One-time Tuition Deposit (make payment online)
  3. One-time Health Clearance form (download)

Housing forms

These forms must be filled-out if you plan to stay in on-campus housing. Please note: on-campus housing is not mandatory for Summer Bridge.

  2. $300 one-time housing deposit for year 2014

Recommended forms:

It is recommended that you fill-out and submit:

  1. FAFSA-Please complete the FAFSA as soon as is feasible if you have not already done so. This will allow us to assist you with financial aid.
  2. AAP Intake Form - Please fax your completed AAP Intake Forms to: (808) 739-4671 (download)
  3. Shuttle request form, if you need transportation from the airport. The deadline is Thursday,
